Clinical History A 16-year-old Caucasian male underwent excsion of a’’ vascular tumour’’ of his left thumb nailbed following a two year history of local swelling. Glomus tumour with microscopic positive margins was diagnosed. Eleven years later, the patient was referred to the Plastic Surgery Department of a different hospital with presumed local recurrence, reporting a painful lesion of the distal left thumb of several months duration that showed dramatic growth and ulceration over the preceding weeks. Malignant transformation was suspected clinically and the provisional plan was amputation of distal thumb the subsequent week. Biopsies were performed ( H&E provided) and showed a malignant round cell tumour in the dermis with focal intraepidermal pagetoid spread.
